Often times even in companies who encourage open collaboration sharing and learning end up conducting these sessions in such a way that blame is ultimately assigned. This leads to staff learning to hold back or defend their positions which in the end leads to a post mortem that is incomplete at best and down right fallacious at worst. This talk is going to guide you through the ways that people think and react during a crisis and how the right approach can be used to ensure you are receiving the best most accurate recommendations from your team once an incident has occurred.</description>
